What you told us, and what changed
- Strategic Plan refresh (2025): a community survey and partner consultations shaped the three strategic directions for 2025–2028, including the commitment to primary care transformation.
- Patient Declaration of Values (2025): written by the Client and Family Advisory Committee and adopted for every partner.
- Website redesign (2026): the current site could not be translated, had no place for partners and no data; this redesign answers each of those.
Come to an event
SOHT staff attend community festivals, ward barbecues and Pride each summer, and partners run pop-up vaccination clinics each fall. See Events.
