Organizations that provide health or community services to people in Scarborough can join the partnership. Membership means a seat at a sector table, a voice in planning, and access to shared tools and funding opportunities — and the responsibilities that go with them.
Who can join
- Hospitals, primary care practices and interprofessional teams, community health centres
- Home and community care agencies, community support services
- Long-term care homes and retirement residences
- Mental health and addictions services
- Rehabilitation, palliative and other specialized providers
- Community and social service organizations whose work affects health
What members commit to
- Take part in their sector table and in implementation teams that touch their work
- Sign the Data Sharing Agreement before exchanging personal health information through SOHT programs
- Apply the Embedding Health Equity Framework in SOHT initiatives
- Report on the outcomes of funded initiatives
How to apply
- Email ScarboroughOHT@shn.ca with your organization, the services you provide in Scarborough and the sector you would join.
- The Secretariat meets you to explain the partnership and answer questions.
- The Governance Committee reviews the application and the Collaboration Council confirms membership at its next meeting.
