The Scarborough Primary Care Network (SPCN) connects, integrates and supports the family physicians and nurse practitioners who care for Scarborough — and gives primary care a voice in how the health system is planned.
What the network is for
- Organize the local primary care sector in Ontario Health Team planning and decision-making.
- Support providers with clinical leadership, peer connection, digital tools (online appointment booking, e-referral, eConsult) and a customer-relationship tool so no practice is invisible to the system.
- Deliver provincial and local priorities: interprofessional primary care team funding, attachment of unattached patients, cancer screening, and a diabetes pathway.
Clinical leadership
Dr. Donna Vlahos and Dr. Rosemarie Lall are the network’s Clinical Co-leads, chosen in 2025 after an open recruitment. They chair the SPCN Governance Committee, whose members practise in comprehensive, specialty-focus, walk-in and team-based settings.
Interprofessional primary care team funding
SOHT coordinates Scarborough’s expressions of interest for provincial interprofessional primary care team (IPCT) funding, using a published decision framework so that the process is transparent to every practice. Round 2 opened in October 2025; documents are in PCN resources.
