SOHT’s members govern the partnership through a Collaboration Council, a Primary Care Network Governance Committee, a Client and Family Advisory Committee and a set of standing committees. The governance structure approved in March 2026 is now being implemented under Strategic Direction 3.
Collaboration Council
The Collaboration Council is the senior decision-making table. Its members are leaders from every sector in the partnership — hospital, primary care, community support services, home and community care, long-term care and retirement homes, mental health and addictions, interprofessional primary care — together with client and family advisors. It sets the strategic directions, approves the operating structure and receives reports from the Operations Committee. It meets every second month.
Primary Care Network Governance Committee
Primary care providers organize themselves through the Scarborough Primary Care Network (SPCN) and its Governance Committee, so that family physicians and nurse practitioners have a voice in OHT decision-making. Two clinical co-leads, Dr. Donna Vlahos and Dr. Rosemarie Lall, provide clinical leadership. Membership is drawn from comprehensive, specialty-focus, walk-in and team-based practice settings so the committee reflects how primary care is actually delivered in Scarborough. Client and Family Advisory Committee
Patients, clients, caregivers and family members co-design SOHT’s initiatives with the implementation teams, review change ideas and take part in the strategic retreat. The Committee wrote SOHT’s Patient Declaration of Values.
Operating structure
Since June 2022 the Collaboration Council has been supported by an Operations Committee (which oversees the sector tables, standing committees and integrated implementation teams), a Governance Committee, a Finance Committee and a Recovery and Sustainability Committee. Sector tables form implementation teams around change ideas — some within one sector, some across sectors — to drive integration. See Core committees for what each one is working on.
Accountability
SOHT is accountable to Ontario Health and the Ministry of Health for the priorities it is funded to deliver, and to its members and community through the Collaboration Council, this website, the partner and community newsletters and public engagement. Members share personal health information only under SOHT’s Data Sharing Agreement, overseen by the Privacy Committee.
